How to Submit a Course Pack Request

Course packs that require copyright permissions must be received in Central Services by the dates published for that semester.

When submitting a request for a course pack you must provide us with the following:

• A completed a Course Pack Order Form

• A clean, neat set of the materials to be copied (call Central Services, x2600 for details)

• The required copyright request information (see below)

Course Pack Submission Schedule

FALL SEMESTER - 2005

          7/19/05   - Submit required forms and photocopies to Central Services for course packs

                                that need Central Services to seek copyright permissions

          8/16/05   - Submit required copying form and photocopies for course packs that do not require

                                 Central Services to seek copyright permissions

                             - Central Services begins printing course packs

          8/30/05  - Central Services sends course packs to the Bookstore for purchase

SPRING SEMESTER - 2006

         11/22/05    - Submit required forms and photocopies to Central Services for course packs

                                  that need Central Services to seek copyright permissions

           1/03/06   - Submit required copying form and photocopies for course packs that do not require

                                   Central Services to seek copyright permissions

                           - Central Services begins printing course packs

            1/17/06   - Central Services sends course packs to the Bookstore for purchase

Required Copyright Request Information

All of the following information is required for each item.

BOOKS

• Title
• Edition Author(s)
• ISBN Number
• Copyright Date
• Copyright Line
• Publisher
• Hard Cover or Trade copy
• Actual Page Numbers
• Total Number of Pages

If the selection is from an anthology, please supply the above information for the original source of publication. (This information can usually be found on the Acknowledgement page.)

MAGAZINE OR JOURNAL

• Title of Magazine or Journal
• ISSN
• Copyright Date
• Copyright Line
• Volume and Edition
• Publisher
• Editor(s)
• Title of Article
• Author(s)
• Actual Page Numbers
• Total Number of Pages
